1 ACCESSIBLE TOBACCO CESSATION PROGRAM (ATCP) An Oral Cancer Preventive Initiative INTRODUCTION Tobacco consumption is a huge public health issue in India and its impact is especially devastating among the poor. Tobacco consumption leads to Non- communicable diseases like Oral cancer (Proportional Mortality Rate-79/lakh), Cardio-Vascular diseases i.e. Ischaemic heart disease and strokes (Proportional Mortality Rate-272/lakh); chronic obstructive pulmonary diseases, cerebrovascular diseases and lower respiratory diseases. These are major public health problem in the Indian subcontinent, where it ranks among the top three for oral cancer in the World. Oral cancer is any cancerous tissue in the oral cavity that could arise either through metastasis from a distant origin or by extension from a neighboring anatomic structures. The most commonly involved site is the tongue followed by floor of the mouth, gingiva, palate, buccal mucosa and lips. The lowincome groups in India are affected most due to a wide exposure to risk factors such as tobacco chewing and insufficient exposure to newly diagnostic aids, resulting in a delay in reporting of oral cancer. Oral cancer is a widespread threat and needs early diagnosis and due monitoring with essential primary health aids. 2 Around 75 percent of oral cancers are linked to modifiable behaviours such as tobacco use and excessive alcohol consumption. Chewing betel, paan and Areca is known to be a strong risk factor for developing oral cancer. Approximately 1 person dies every 6 seconds due to tobacco, accounting for one in 10 adult deaths. Unless urgent action is taken, the annual death toll could rise to more than eight million by 2030. In India where such practices are common, oral cancer represents upto 40% of all cancers. Effective tobacco control should be a top priority, both as a health issue and as a method to reduce poverty. As the proverb goes- “Prevention is better than cure” We will follow an Integrated approach to help tobacco users quit by educating and creating awareness about the harmful effects of Tobacco intake through our Accessible Tobacco Cessation Program (ATCP) which are easily accessible due to their mobile/portable nature in functioning as their Modus Operandi. The therapy will involve individual or group counselling and may include the dispensing of pharmacological aids, if equipped with. SOCIAL IMPACT The global trends of oral diseases are witnessing a paradigm shift with the rising numbers of oral cancers. Dental Health Care Providers have an immense potential in preventing and controlling this rise that needs to be channelized strategically. There will be consistent identification, documentation and treatment of every tobacco user at each visit to the Accessible Tobacco Cessation Program. Effective treatment will be offered to all tobacco users. Consequently, each Accessible Tobacco Cessation Program would be “An Initiatives for prevention of Non-communicable diseases caused by tobacco consumption”. INNOVATIVENESS Tobacco Control Program recognized tobacco cessation as an important component and a critical step in controlling tobacco’s Proportional Mortality Rate, cessation activities have still not kept pace with other tobacco control activities. Less than half of the states under the program have established tobacco cessation facilities at a district level. 3 Inspite of different measures taken by Indian Government and WHO, we need to think of additional strategies to reach out to more tobacco users in a more sustainable and cost effective way. Accessible Tobacco Cessation Program (ATCP) is a new concept in India to reach substance users (tobacco, alcohol and illicit drug users) in an easily accesssible way and also changing lives majorly. SCALABILITY ATCP can achieve growth by replicating their concept within Delhi and every state of India. Business and Social Impact Plans Mission Statement: The mission is to minimise the Proportional Mortality Rate caused by substance use with the help of Accessible Tobacco Cessation Program (ATCP).
