The blood–brain barrier (BBB) is a highly selective interface that protects the central nervous system (CNS) from toxins, pathogens, and unwanted macromolecules. However, this protection creates a major challenge for neurotherapeutics: over 98% of brain-targeted molecules are actively expelled by efflux transporters such as P-glycoproteins and multidrug resistance proteins. This limits treatment options for conditions including glioblastoma, spinal cord injury, cerebral ischemia, and rare neurological disorders. To overercome these challenges, we are designing novel therapeutic small molecules, peptides, antibody-drug conjugates and delivery platforms which can breach the blood brain barrier, blood cerebrospinal fluid barrier as well as blood brain tumor barrier.
