Generally, when a patient visits a hospital or a clinic for treatment a certain set of information is required by the respective doctor or consultant such as symptoms information, behavioral information, past medication, and various laboratories value for the scientific evaluation of patients’ profile and to plan treatment protocols accordingly. Conventionally, the above information i.e., symptoms information, behavioral information, past medication, and various laboratory values are provided by the patient to the doctor or his staff. The concerned doctor analyzes the above information to evaluate the patient’s profile and plan treatment protocols accordingly. However, in some cases, the patient is not able to explain his or her problem due to the following reasons  privacy issues  Phsycological pressure before the doctors  Concerned doctors have Limited time assigned per patient.  Any minor mistake in the information provided by the patient more specifically in case of emergency will severely affect the planning for treatment protocols. Therefore, the existence of an effective system is required in a clinic or hospital in which the patient will provide all the information i.e., symptoms information, behavioral information, past medication, and various laboratories’ value and analysis of the same which help the doctor or consultant for quick decision making related to the treatment of patient i.e., analyzing the treatment results and to plan treatment protocols accordingly.
