In their own words
Mental Health is a state of well-being in which an individual realizes his or her own abilities, can cope with the normal stresses of life, can work productively and is able to make a contribution to his or her community. It is an integral and essential component of “Health”. Mental health is more than just the absence of the mental disorders or the disabilities. The progress and prosperity of a nation depend on the overall quality of health of its citizens. However, until the early twentieth century the issues of mental health and well-being were inadequately addressed.Since independence, various initiatives have been taken by individuals and by the central government and the state governments, to develop the infrastructure of the basic health facilities. Development of the mental health services in India has also received an impetus. Unfortunately, mental health services in India have not yet matured. While the National Mental Health Programme (NMHP) of 1982 and 2003 amendment has created a wide variety of mental well-being initiatives based on the needs of the community, these initiatives are largely limited in their access. Mental health continues to invite a lot of stigma, because of which the data on mental health remains inadequate.Mental disorders are among the leading causes of the non-fatal disease burden in India, but a systematic way of understanding their prevalence, disease burden, and risk factors is not readily available for each state of India. This lack of data itself is indicative of the lack of the political will and the overall lack of information about the state of mental health in India. In 2020, India State-Level Disease Burden Initiative Mental Disorders Collaborators published a report in the highly reputed journal Lancet’s psychiatry edition and reported the following are:• In 2017, 197·3 million people had mental disorders in India, including 45·7 million with depressive disorders and 44·9 million with anxiety disorders.• The contribution of mental disorders to the total disability-adjusted life-years (DALYs) in India increased from 2·5% (2·0 – 3·1) in 1990 to 4·7% (3·7 – 5·6) in 2017.• In 2017, depressive disorders contributed the most to the total mental disorders DALYs (33·8%, 29·5 – 38·5 years), followed by anxiety disorders (19·0%, 15·9 – 22·4 years), idiopathic developmental intellectual disability (10·8%, 6·3 – 15·9 years), schizophrenia (9·8%, 7·7 – 12·4 years), bipolar disorder (6·9%, 4·9 – 9·6 years), conduct disorder (5·9%, 4·0 – 8·1 years), autism spectrum disorders (3·2%, 2·7 – 3·8 years), eating disorders (2·2%, 1·7 – 2·8 years), and attention-deficit hyperactivity disorder (ADHD; 0·3%, 0·2 – 0·5 years); other mental disorders comprised 8·0% (6·1 –10·1 year).• One in seven Indians were affected by the mental disorders of varying severity in 2017.• The proportional contribution of the mental disorders to the total disease burden in India has almost been doubled since 1990.It is evident that the burden of the mental health diseases and disorders is increasing and the response – from both the government and the private healthcare sectors is not commiserated.
