Access to quality primary care in a regular and reliable manner is a big challenge for the Indian population living in the vast rural hinterlands. Given a major distribution imbalance, the urban population (28% of total) enjoy access to over 66% of the country’s hospital beds, while the remaining 72% in rural areas have just 33% of the beds to take care of their needs. Insufficiencies in publicly provided primary healthcare services coupled with economic & logistic barrier to urban healthcare and lack of health education has two effects. It drives people across socio-economic strata to turn to private healthcare facilities. However, the cost of treatment at private healthcare facilities is at least 2 to 9 times higher than at public facilities. This leads to ‘Health-related poverty’. Our hypothesis of a possible sustainable solution to this conundrum revolves around - certified training of rural youth as 'Community Health Workers' and equipping them with a data driven 'Decision-support’ technology platform and frugal innovative diagnostic technologies that to integrated with the software ecosystem. The software enables the CHW to extract structured history from the patient based on the symptoms and guides the CHW to undertake a limited physical examination. The data is sent to the back-end doctor at a remote location using low bandwidth internet or nested LoRa devices for evaluation. The patient's clinical management is guided by the doctor and all data get stored in a HIPAA compliant cloud server. The EHR (Electronic Health Record) becomes a very important source of health data of a large number of people. It will incorporate data analytics engine that, through AI-ML based modelling can predict community spread, create heat maps, identify at-risk groups, link with government services, disseminate accurate information (thus preventing both mis-information and information overload). The resulting structured clinical data will also lend itself to valuable public health research and policy formulation.
